One of Northern California's best and most visited hospitals is looking for an RN Case Manager to join their wonderful team of nurses.
This 80
bed hospital has been serving their hospital for over 100 years.
The Case Manager will join a medical center that believes it's important to treat the patient wholly and provide spiritual care as well as medical.
Reporting directly up to the Director of Case Management, Case Manager will work alongside other case managers and nurses daily.
Previous experience in a similar role is required for consideration.
This hospital is looking for a Case Manager who can take charge of Case Services for Primary Care and Ambulatory Clinics.
This RN Case Manager will report to the Clinic Site Administrator.
The RN Case Manager will:
Provide transitional and chronic care management.
Identify eligible patients, advocates for enrollment, assess patient needs, and barriers to care.
Develop goals, monitor outcomes, and evaluate patient readiness for graduation from programs.
Improve health of populations, improve the patient experience, reduce readmissions, reduce utilization of higher cost healthcare.
Coordinate with Primary Care Providers and all members of the healthcare team.
Work with patients and their family members.

$83 per hour.
Estimated Salary: $20 to $28 per hour based on qualifications.
